Upon Arrival: You will be greeted by one of our friendly staff members here at Sun Valley Urgent Care. We strive to make your visit as convenient as possible. There is no appointment necessary to be seen at Sun Valley Urgent Care. Required documentation to bring: - A current photo I.D. when you check in at the front desk is required for all patients. ie: driver's license, passport, or a military I.D. - A copy of your current insurance I.D. card. Patients under 18 years of age: Patients under the age of 18 must be accompanied by a parent or a legal guardian. We see patients 4 months and older. Forms of Payment Accepted: - Visa - MasterCard - Debit Cards - Cash Insurance Plans: Most health insurance plans are accepted. We gladly accept uninsured patients; please call for pricing.
